where do referrals come from?
The attending physician, social worker, discharge planner, clergy, patient, family member, friends or any person concerned with a patient’s well-being can make a referral. And our team of professionals will coordinate with the patient’s physician for approval of services.
how are services paid?
Palliative Care and Hospice Services are covered by Medicare, Medi-Cal, and most private insurance plans. Certain medical and eligibility requirements need to be met. Applied Palliative and Hospice Services, Inc., will work with the patient’s insurance companies to coordinate care and benefits.
